As the global community marked World Tuberculosis (TB) Day on the 24th March 2024 with the theme "Yes! We can end TB!", conveying a message of hope that getting back-on-track to turn the tide against the TB epidemic is possible through high level leadership, increased investments and faster uptake of new WHO recommendations. Lesotho found itself at the forefront of the battle against this pervasive disease.

World Vision International Lesotho (WVI), in collaboration with the Ministry of Health, took proactive steps towards ending TB and improving health outcomes for vulnerable populations through innovative approaches and community-driven initiatives.

TB remains a pressing concern in Lesotho, affecting 14% of the population, including 1,100 children. Alarmingly, 26% of TB patients, including 909 children, are lost to follow-up, posing significant challenges in managing the spread of the disease. Recognizing the urgent need for action, World Vision's Health, HIV, and Nutrition Programme implemented a comprehensive strategy to raise awareness, educate communities, and provide integrated health services.

One of the critical challenges in TB prevention has been the lack of knowledge and delayed diagnosis, particularly among children. Children under five years old face difficulties in producing sputum for diagnosis, making X-rays the preferred diagnostic modality. However, access to X-rays is limited to hospitals, posing barriers for vulnerable communities.

External factors exacerbating the TB problem include the geographical distance of health facilities from communities, lack of resources such as sputum induction machines for children, migration to South Africa, and misconceptions about TB that lead to poor health-seeking behaviors.

World Vision's holistic approach, which integrates TB screening with other essential health services, promotes early detection and treatment initiation, thereby reducing TB incidence and improving overall community health outcomes.

Through partnerships with Village Health Workers and health facilities, World Vision ensures community engagement and participation in TB and non-communicable disease prevention and treatment efforts. This community-led approach fosters sustainable change and promotes health equity.
